Before Your Next Appointment
Before you call, please have this information ready for the receptionist:
- Do you need a mammogram? Annual mammogram screening is recommended for women 40 years old and older. Please let the receptionist know where you would like the order for your mammogram to be sent.

Do you need a bone density scan? This brochure outlines Osteoporosis and its risk factors. Fill out the Osteoporosis Risk Assessment. You can discuss the answers with the doctor or nurse and determine whether a bone density scan is necessary for you.
If you do need to schedule a bone density scan, please keep these few things in mind. First, not all insurance carriers cover this service, so be sure to check with your insurance company to verify your benefits before your appointment. Second, on the day of your appointment, be sure to dress comfortably and in clothing without buttons, snaps or zippers, as these may interfere with the scan.
- Pap smears should be scheduled 10-14 days from the start of your cycle. Please have this information available when calling to schedule. Three (3) days before your Pap smear, you should have nothing in the vagina: no intercourse, no tampons, no douching.
- Lipid profile screening is required for non-smoking women 35 years old or older who are using a birth control pill, patch or ring. This test is also available to all patients who are interested in screening. The test requires a 12 hour fast - no food for 12 hours prior to the test. You should drink water in order to stay hydrated.
- Please obtain a referral for your appointment if it is required by your insurance plan.
Preparing for your appointment:
- If you are a new patient, please download and complete all of the New Patient Forms and bring them with you to your appointment. We ask that new patients arrive 15 minutes early for their first appointment. You will also need to bring your current insurance card, a photo ID and a valid credit card (see Credit Card Policy).
- If you are a returning patient coming in for your annual check-up, please download the Returning Patient Forms, complete and bring them with you to your appointment. Please also bring your current insurance card, photo ID and valid credit card so we can update your records.
- Co-pays are due at the time of service. We will bill your insurance for your visit. If you do not have insurance, payment in full for the visit will be due at the time of service, unless previous arrangements are made. We accept Cash, Check or Credit Card (Visa, MasterCard & Discover).
Canceling an Appointment / No Show Policy:
- Missed Appointments: If you are unable to keep an appointment, we ask that you kindly provide us with 24 hours notice. This courtesy on your part will make it possible to give your appointment to another patient who needs to see the doctor. Failure to provide this notice takes appointment times that could have been given to other patients and will be considered a “No Show.” Our phone lines allow you to leave messages any time of day or night, so please call with your earliest knowledge that you must miss or reschedule an appointment. If you must cancel an appointment, we will do our best to reschedule your visit as soon as possible.
- “No Show” Policy: If you do not call to cancel or reschedule an appointment, it will be documented as a “no show” in your chart and in our computer system. We will notify you by letter each time you have a no show. Our policy states that if a patient has three no shows, she is unable to schedule further appointments and will be discharged from our practice.
