Insurance and Fees

Thank you for choosing us as your health care provider. We are committed to your treatment being successful. Please understand that payment of your bill is considered a part of your treatment.

We will bill all participating insurance companies. You are responsible for the balance whether your insurance pays or not. We cannot bill the participating insurance companies unless you bring in all insurance information. Your insurance policy is a contract between you and your insurance company. We are not a party to that contract. Please be aware that some services provided may be non-covered services and not considered reasonable and customary under your insurance plan.

Patients who are submitting claims themselves and/or patients who are without insurance coverage (self-pay) must pay in full at the time of service unless other arrangements are made.

Our practice is committed to providing the best treatment for our patients and we charge what is reasonable and customary for our care. You are responsible for payment, regardless of any insurance company’s arbitrary determination of reasonable and customary rates.
